  Can we assume that input will always be a translation from a higher-level
  language?

No.

   Or could input be any sequence of CPU instructions?

Yes. Remember, the answer to the question "Can the input be..." is always
"yes". Of course, you can work yourself into philosophical conundrums this
way:

  Can the input be always correct?

  Yes.

  Can the input be always incorrect?

  Yes.

  But, but... if the input's always correct then... it can't be always wrong...
  (smoke begins to pour out of the ears).

Fortunately, this isn't a philosophy class (or an episode of Star Trek where
the crew of the Enterprise is being menaced by sentient computer programs), so
this issue isn't of interest to us.
