Are there any requirements for the mud cards besides a question and a name?
Can they be an index card, a slip of paper, or any other stationery?

A mud card has four fields:

1 Your name, or your student id.

2 The book from which the question comes. This should be written out in
full (title + authors) the first few times for each book you're using.
After that, you can use the authors. If you're using the recommended
      text, "SGG" is fine.

3 The section from which the question comes. The more specific the
      location, the better.

4 The question, which should read like a question; for example, "The
concept of an operating system" is not a question, it's a phrase. The
question should be specific to a particular context in the reading; for
example, "What is the concept of an operating system?" reads like a
question, but is probably too general (depending on context).

The card is a piece of paper; "card" suggests small size (3x5). Writing
  should be legible.

Mud cards start in ernest next Monday (2 February).
