I am assuming that you will want to have both soft copy and hard copy of the
code (with appropriate documentation, of course).
No; everything you hand in will be handed in via e-mail. You should have
gotten a message describing where you can get more information; if not, check
out the hyper-mail archive.
I am of the minimalist school of documentation; if you follow Section 1.6 in
Kernighan and Pike, you'll be in good shape.
Generally, I'd prefer that you just submit source code without any extra
documentation files. If you have a gripping tale to tell, you can make it a
header to one of your files. If you absolutely must include extra
documentation files, make them part of your submission; as long as they aren't
named like .cc or .h files, they will be ignored by the mailbox software (which
means you have to use the send-files command line to send them, otherwise
they'll be ignored).
