New Jersey State Law requires all matriculating students born 1/1/57 or later to provide documentation of immunity to measles, mumps and rubella. Acceptable records include physician-signed records, official school or college documents, parent's immunization records (if signed by MD), and public health records. In the event of a vaccine-preventable disease outbreak, students who fail to provide proof of immunity may be temporarily excluded from classes. In addition to immunization records, residential students and athletes are to submit to Health Services a complete Student Health Record which includes a medical history and current physical (no less than two months old for student-athletes). These records are to arrive at the Health Center prior to occupancy in residential halls and/or participation in sports. Students will be informed of missing information upon receipt of these records. Students who do not provide this information will jeopardize their future registrations. Transfer students click below to obtain the Transfer Student Release Form to be submitted to their former college/university. Re-enrolling students should contact the Health Center to ascertain the status of their records. The American College Health Association (ACHA) and the U.S.Centers for Disease Control and Prevention recommend tuberculin testing for students from foreign countries. International students, in addition to immunization records (if applicable), must provide proof of tuberculin testing (Mantoux) within the previous year, regardless of BCG status. Positive test results require proof of chest x-ray. Records are to be in English. ACHA recommends that college students
consider vaccination against meningococcal disease. (See their website

In recent years, the incidence of serogroup C meningitis has risen sharply
on many college campuses. College students have a greater risk of
outbreak than the general population due to increased risk factors such
as community living, upper respiratory infections, active and passive smoking
and bar patronage. In response to ACHA's recommendation Health Services
is urging you to discuss this issue with your physician.
